Sometimes hospice patients need care beyond what can be provided at home. For qualified patients needing acute medical management of symptoms or respite when their caregiver needs a break, Heart to Heart Hospice offers access to care in our hospice inpatient facilities. Our compassionate team of physicians, nurses, and hospice aides focus on bringing the patient’s symptoms under control with the goal of getting them back home. On-site social workers and chaplains support both patient and their loved ones emotionally and spiritually. Each hospice inpatient center provides a warm, home-like environment, designed to welcome family and friends. Overnight accommodations for one family member, private bathroom and shower with 24-hour access are hallmarks of our hospice centers. If placement is needed for “peace of mind’, ask us if your loved one qualifies for routine hospice care with a private pay option for room and board.
